Maintaining normothermia during transport has its challenges though, and a 2023 study found up to one third of infants arriving at the destination hospitals are hypothermic<sup>1<\/sup><\/p>\n\n\n\n<figure class=\"wp-block-image aligncenter size-full\"><img loading=\"lazy\" decoding=\"async\" width=\"1000\" height=\"600\" Issue<\/h2>\n\n\n\n<p>Neonates are particularly vulnerable to <a href=\"https:\/\/campusvygon.com\/global\/hypothermia-in-newborns-how-to-prevent-it\/\" target=\"_blank\" data-type=\"link\" data-id=\"https:\/\/campusvygon.com\/global\/hypothermia-in-newborns-how-to-prevent-it\/\" rel=\"noreferrer noopener\">hypothermia <\/a>due to their limited ability to regulate body temperature effectively and the fact that they can also be exposed to prolonged low temperatures. &nbsp;Hypothermia is an important factor in morbidity and mortality of all birthweights and gestational ages, and particularly for vulnerable preterm infants<sup>2<\/sup>.&nbsp;<\/p>\n\n\n\n<p>Newborn admission temperature is a strong predictor of outcomes across all gestations<sup>3<\/sup>&nbsp;as hypothermia can lead to a cascade of adverse effects, including respiratory distress, metabolic acidosis, and compromised immune function.<\/p>\n\n\n\n<figure neonates<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">International recommendations<\/h2>\n\n\n\n<h3 class=\"wp-block-heading\">Interventions to prevent hypothermia at birth in preterm and\/or low birth weight infants<\/h3>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p><em>Use of an occlusive polyethylene transparent wrap or bag immediately at birth improved admission temperatures in non-randomised studies of infants at &lt; 27 weeks\u2019 gestation [\u2026], between 28 and 30, weeks [\u2026] and at &lt; 33 weeks\u2019 gestation [\u2026], as well as in very low birth weight infants and extremely low birth weight infants when compared with routine care<\/em><\/p>\n\n\n\n<p><strong>Cochraine Database of systematic reviews \u2013 2018<\/strong><\/p>\n<\/blockquote>\n\n\n\n<h3 class=\"wp-block-heading\">Improvement in thermoregulation outcomes following the implementation of a thermoregulation bundle for preterm infants<\/h3>\n\n\n\n<blockquote class=\"wp-block-quote is-layout-flow wp-block-quote-is-layout-flow\">\n<p>The key practice changes included: improved anticipation and staff preparedness, wrapping infant in a polyethylene sheet, using a polyethylene lined bonnet, using servo-control mode at birth and during transport.<\/p>\n\n\n\n<p><strong>Journal of Pediatrics and Child Health \u2013 2022<\/strong><\/p>\n<\/blockquote>\n\n\n\n<p>When addressing hypothermia prevention, it is essential not to overlook thermal management during newborn transport. It may be \u00ab&nbsp;replaced&nbsp;\u00bb by [\u2026] <strong>a transparent polyethylene bag<\/strong> wrapping whilst the baby is still wet. This greatly <strong>reduces the risk of hypothermia<\/strong>\u201d<sup>7<\/sup><\/li>\n\n\n\n<li>\u201cThe implementation of&nbsp; <strong>Velcro-closure polyethylene wrap <\/strong>was a helpful adjunctive tool that allowed a higher coverage of body surface area [\u2026]\u201d<sup>5<\/sup><\/li>\n<\/ul>\n\n\n\n<p>Therefore, neonatal transfer services may implement the use of polyethylene plastic bags during transport to optimize thermal management in neonates.<\/p>\n\n\n\n<div style=\"height:50px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\">The Importance of Preparation and Planning:<\/h2>\n\n\n\n<p>Nevertheless, effective thermal management during transport is part of a wide strategy and requires a multi-faceted approach:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Anticipate the need for thermal support based on the infant\u2019s gestational age and condition.<\/li>\n\n\n\n<li>Ensure that transport equipment is pre-warmed and readily available.<\/li>\n\n\n\n<li>Develop standardised protocols for thermal management during transport.<\/li>\n<\/ul>\n\n\n\n<p>Optimal thermal control during neonatal transport requires a coordinated effort from all members of the healthcare team. This includes:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Effective communication between referring and receiving facilities.<\/li>\n\n\n\n<li>Thorough training of transport personnel in thermal management techniques.<\/li>\n\n\n\n<li>Continuous quality improvement initiatives to monitor and enhance the effectiveness of thermal management protocols.<\/li>\n<\/ul>\n\n\n\n<p>In conclusion, admission hypothermia can be reduced through the implementation of appropriate measures starting from the initiation of newborn transport.
