endovenosa e scelta del catetere appropriato"},"content":{"rendered":"\n<p>La terapia infusionale ci consente di somministrare farmaci o liquidi direttamente nel circolo ematico, attraverso un dispositivo vascolare posizionato in una vena periferica o centrale. <\/p>\n\n\n\n<p>Questo ci consente di ottenere:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>un rapido assorbimento del farmaco;<\/li>\n\n\n\n<li>Il mantenimento di un dosaggio costante nel tempo;<\/li>\n\n\n\n<li>somministrazione di farmaci in bolo per un effetto immediato.<\/li>\n<\/ul>\n\n\n\n<p>Le sostanze infuse hanno delle caratteristiche fisico-chimiche (osmolarit\u00e0, PH) che possono renderle potenzialmente dannose per l\u2019endotelio dei vasi.<\/p>\n\n\n\n<div style=\"height:42px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Osmolarit\u00e0<\/strong><\/h2>\n\n\n\n<p>Esprime la concentrazione di una soluzione e ne determina la sua pressione osmotica, sapendo che la pressione osmotica fisiologica del corpo umano \u00e8 pari a 280 mOsm\/kg, valori che si discostano, per eccesso o per difetto, risultano potenzialmente dannose per i vasi sanguigni.<\/p>\n\n\n\n<div style=\"height:26px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<figure class=\"wp-block-image aligncenter size-large is-resized\"><a rischio<\/strong><strong><\/strong><\/h2>\n\n\n\n<p>In base ai valori di osmolarit\u00e0 e PH distinguiamo 3 classi di rischio:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li><strong>Basso rischio<\/strong>: osmolarit\u00e0 &lt; 450 mOsm\/l, ph 5-7 e farmaco non vescicante;<\/li>\n\n\n\n<li><strong>Rischio moderato:<\/strong> osmolarit\u00e0 450-600 mOsm\/l, o ph 4-5 o 7.5-9 e farmaco non vescicante;<\/li>\n\n\n\n<li><strong>Rischio alto:<\/strong> osmolarit\u00e0 &gt; 600 mOsm\/l, ph &lt; 4 o &gt; 9, o farmaco vescicante.<\/li>\n<\/ul>\n\n\n\n<div style=\"height:41px\" class=\"wp-block-heading\"><strong>Emodiluizione<\/strong><\/h2>\n\n\n\n<p>Il principio cardine sul quale basare la scelta del catetere vascolare appropriato ad una determinata terapia farmacologica \u00e8 l\u2019emodiluizione, ossia la diluizione che il farmaco riceve appena raggiunge il torrente circolatorio. <\/p>\n\n\n\n<p>La tolleranza umana del PH e dell\u2019osmolarit\u00e0 non \u00e8 stata ben studiata, ma \u00e8 dato certo che maggiore sar\u00e0 il flusso venoso, maggiore sar\u00e0 la diluizione che il farmaco ricever\u00e0 al suo ingresso nel vaso, maggiore sar\u00e0 la tolleranza dell\u2019endotelio del vaso a:<\/p>\n\n\n\n<ul class=\"wp-block-list\">\n<li>Valori di PH estremi<\/li>\n\n\n\n<li>Osmolarit\u00e0 elevate<\/li>\n\n\n\n<li>Sostanze lesive di altra natura<\/li>\n<\/ul>\n\n\n\n<div style=\"height:45px\" aria-hidden=\"true\" class=\"wp-block-spacer\"><\/div>\n\n\n\n<figure class=\"wp-block-image aligncenter size-large is-resized\"><a venoso maggiore viene raggiunto all\u2019interno della vena cava superiore, che risulter\u00e0 quindi il sito pi\u00f9 sicuro nel quale infondere farmaci e soluzioni.
